Resurfacing is the right word for what happens. The laser removes the damaged surface in a controlled grid and prompts the skin to rebuild it. That is a different proposition from the non-ablative devices that warm the dermis and hope for a collagen response, and the results and the recovery both reflect the difference. If you have tried gentler treatments and found the improvement never quite held, this is usually why.

What fractional ablative actually means

Two words carry the meaning. Ablative means the laser vaporizes tissue rather than heating it from underneath. That is what gives it reach that non-ablative devices do not have, and it is also why the recovery is real. Fractional means it treats a grid of microscopic columns instead of the whole surface, leaving untouched skin between them to drive healing. Full-field CO2 resurfacing in the 1990s removed everything and put people out of commission for a month. Fractionating the beam brought that down to about a week. The American Society for Dermatologic Surgery covers the general principles of laser resurfacing if you want independent background.

The CO2RE runs four fractional modes. Light works through the epidermis as a refresh. Mid drives deeper for moderate photodamage and lines. Deep pushes narrow columns well into the dermis, which is the setting for scarring and heavy resurfacing. Fusion combines a superficial pattern with a deep spike at the centre of each column and treats both layers in a single pass. All four are fractional. Depth is what changes, and so does your recovery.

Why we finish every treatment with exosomes

When the laser pass ends, your skin is covered in thousands of open micro-channels running into the dermis. They close quickly. That short window is the only moment when something applied topically can actually reach the depth where healing is being decided, and most protocols waste it.

We use it. Immediately after the pass we apply exosomes into those channels. Exosomes are extracellular vesicles carrying growth factors and signalling proteins, and delivering them into open channels rather than onto intact skin is what makes the difference between a treatment step and an expensive serum. The intent is to calm the inflammatory response, support the repair happening in the dermis, and shorten the crusting and peeling phase.

What that means in practice: the downtime is still mandatory and you should still plan for it. Our patients typically describe the redness settling sooner and the whole process being more tolerable than the accounts they have read of standard CO2 recovery. We would rather set that expectation honestly than promise you a number.

What we treat, and what we send elsewhere

CO2 resurfacing earns its place on damage with depth to it:

  • Deep, etched wrinkles that stay visible at rest, particularly around the mouth and eyes.
  • Severe sun damage where the skin has thickened and roughened, not isolated marks on the surface.
  • Coarse texture and enlarged pores.
  • Deep acne scarring and mature surgical or traumatic scars.

It is the wrong tool for plenty of things, and a week of downtime is a steep price for a problem a gentler device handles well. We will send you elsewhere if your concern is scattered surface spots or freckling on otherwise healthy skin, which is what our IPL treatments are built for. Facial redness and flushing belong to vascular laser. Dullness and mild unevenness respond to a course of chemical peels with no downtime at all. Early texture change is better met by microneedling or RF microneedling. Being told you do not need the expensive laser is a legitimate outcome of a consultation.

Are you a candidate?

The people who do best with CO2 resurfacing have visible, structural damage rather than early concerns, realistic expectations about what one treatment achieves, and a calendar with room in it. That last one matters more than people expect. Someone who cannot protect a week of recovery, or who knows they will be back outdoors in the sun immediately, will get a worse result from this laser than from a treatment they can actually accommodate.

Skin tone matters too. Deeper Fitzpatrick types carry a genuine risk of post-inflammatory hyperpigmentation with ablative lasers, and the honest answer is often a modified conservative approach with pre-treatment, or a different treatment entirely. Who should avoid CO2 laser skin resurfacing?

You are not a candidate for CO2 laser resurfacing, or will need medical clearance first, if any of the following apply:

  • Pregnancy or breastfeeding.
  • Active infection, an open cold sore, or inflamed acne lesions in the treatment area.
  • Recent isotretinoin. The doctor will tell you how long to wait.
  • A history of keloid or hypertrophic scarring. The treatment creates a controlled wound, and your skin has already shown how it responds to wounds.
  • Deeper skin tones, which carry real post-inflammatory pigmentation risk and need either a conservative modified approach with pre-treatment or a different treatment.
  • Conditions or medication that impair wound healing, including immunosuppression, poorly controlled diabetes, and some autoimmune and connective tissue disorders.
  • Recent sun exposure or an active tan in the treatment area.
  • An inability to commit to strict sun avoidance through the healing period.

Essential aftercare after CO2 resurfacing

Aftercare is not optional here, and it is not light. What you do in the first ten days determines a good part of your result:

  • Vinegar or saline soaks. Dilute white vinegar or sterile saline soaks several times a day through the weeping and crusting phase, exactly as we prescribe. They keep the surface clean, lift debris gently, and lower infection risk.
  • Heavy occlusive barrier. Aquaphor or the equivalent, applied thickly and reapplied through the day and night. Healing skin has no barrier of its own and must not be allowed to dry out. Nothing with acids, retinoids, vitamin C, fragrance or exfoliants until we clear you.
  • Do not pick. At all. Not the crusts, not the flakes, not the piece that looks ready to come off. Picking is the most common cause of a bad outcome from this treatment.
  • Absolutely zero sun exposure. Not reduced. Zero, through healing, then rigorous SPF 50 and a hat for months afterwards, in line with Canadian Dermatology Association sun protection guidance.
  • Sleep elevated for the first few nights to limit swelling.
  • No exercise, saunas, hot showers, swimming pools or alcohol while you are healing. Heat and blood flow make the swelling worse.
  • No makeup until the peeling has finished and we have cleared you.

Safety and side effects

Expected effects of CO2 laser skin resurfacing, which are part of the treatment rather than complications:

  • Intense heat and tightness for the first several hours.
  • Redness and swelling peaking in the first 48 hours, often worst around the eyes on waking.
  • Clear or yellowish weeping in the first day or two.
  • Bronze or brown micro-crusting, then heavy uneven peeling.
  • Pink or ruddy tone lasting weeks to a few months after deeper treatments.
  • Temporary itching as the skin knits together.

Less common risks, which is why this is run as a medical procedure: post-inflammatory hyperpigmentation, particularly in deeper skin tones or with any sun exposure during healing; infection, including reactivated cold sores, which is why antiviral cover is prescribed; prolonged redness; and scarring, which is rare but real and is most often linked to picking or to infection during healing. Call us rather than waiting if anything looks or feels wrong.

Protecting your result long term

The laser starts the process. What you do afterwards decides how long the result lasts:

  • Daily broad-spectrum SPF 50, permanently. Ultraviolet exposure is what caused most of the damage we just corrected. Skipping this undoes the work.
  • Retinoids once we clear you, usually several weeks out, to keep collagen turnover going.
  • Antioxidants in the morning under sunscreen.
  • Lighter maintenance in between, whether that is peels, microneedling or a Light-mode CO2 pass, rather than waiting years and needing another deep session.
  • Judge the result at three to six months, not at two weeks. Collagen remodelling runs long after the redness fades.

Deep wrinkles and etched lines

Lines that stay visible when your face is at rest have been carved into the dermis, and no cream reaches them. CO2 resurfacing removes columns of that damaged tissue and drives new collagen into the gap as it heals. The perioral lines around the mouth and the crepey skin under the eyes respond particularly well, because those are the areas where nothing non-ablative has enough reach to help.

Severe sun damage and photoaging

We are talking about skin that has structurally changed after decades of exposure: thickened, leathery, roughened, with pigment sitting on top of damaged texture. Treating the colour alone leaves the surface unchanged, which is why spot devices disappoint on this kind of skin. Resurfacing rebuilds the surface and the pigment improves along with it.

Texture, pores and overall skin quality

Rough patches, enlarged pores, and the general dullness of skin that no longer reflects light evenly. This is the full-field use of the laser, where the aim is the quality of the whole surface rather than one specific mark on it. It is also the work that makes makeup sit differently afterwards, which is what most people actually notice first.

Laxity and loss of firmness

The collagen response continues for months after the treatment, and that is where the tightening comes from. CO2 resurfacing will not replace a facelift, and anyone promising that is selling you something. On mild to moderate laxity it firms the skin meaningfully. Consultation with the doctor

You complete the intake and consent forms, then meet our medical director. She examines your skin, goes through your medications and history, and decides whether an ablative laser is the right instrument for what you want corrected. Recent isotretinoin, active infection, a history of keloid scarring, or a deeper Fitzpatrick skin type all change the plan or rule the treatment out. If you are cleared she writes the prescription and sets the treatment parameters. In Quebec that step is the law, not a formality.

Medical-grade numbing

Comfort is planned, not improvised. On treatment day the nurse cleans and photographs the area, then applies a medical-grade topical anaesthetic cream and leaves it to work for around 45 minutes under occlusion. You will feel heat and a prickling snap during the pass, but the numbing plus the device cooling keeps it well inside what people tolerate. Build the extra 45 minutes into your day, and arrive without makeup so we are not spending it cleansing.

The laser pass

Our specialized cosmetic nurse performs the treatment at the settings the doctor has set, choosing depth and density to match what is being corrected. A full face takes roughly 30 to 45 minutes of laser time. You will smell something during the pass, and afterwards the skin looks and feels like a strong sunburn. Arrange your ride home in advance.

Plan for 7 to 10 days of social downtime for the deeper resurfacing settings. Not two days, and not a long weekend. Light-mode resurfacing is a shorter proposition, closer to three or four days before makeup will sit properly, and Mid sits between the two. Ask which mode you are booked for and plan against that number rather than the best case.

Our exosome protocol makes that window shorter and considerably more comfortable than standard CO2 recovery, but it does not remove it. Book against a quiet stretch of your calendar, and take the time off before you book rather than after.

Days 1 to 3. Significant redness and swelling, and a sensation like a severe sunburn. The heat is most intense in the first few hours. Swelling peaks in the first 48 hours and is usually worst around the eyes when you wake up. The skin weeps a clear or yellowish fluid. You will be doing soaks and reapplying ointment constantly, and you will not want to be seen.

Days 4 to 7. The weeping stops and the surface dries into fine bronze or brown micro-crusting with a sandpaper feel. The skin darkens before it clears, which surprises people. Then heavy peeling begins, coming away in uneven patches with bright pink new skin underneath. This is the stage people find hardest to look at, and it is also the stage where the exosomes earn their place.

Week 2 onward. Peeling finishes and the pinkness starts fading. Most people are covering what remains with mineral makeup and back to normal life. A pink tone can persist for several weeks to a few months after a deep treatment, which is normal.

Months 1 to 6. The actual result builds as collagen remodels. What you see at two weeks is not the outcome.

You risk a permanent scar and you risk infection. The crust is a temporary barrier over a wound that has not closed. Pulling it off early tears skin that is still knitting together, and picking is the single most common cause of a poor outcome from CO2 resurfacing.

It will itch, sometimes intensely, and it will look ready to come off before it is. Keep it greasy, since ointment reduces the itch, and let every piece fall away on its own. If the itching becomes unbearable, call us instead of touching it.

Absolute. Zero direct sun while you are healing, then rigorous protection for months. New skin after ablative resurfacing has almost no defence against ultraviolet light, and exposure causes post-inflammatory hyperpigmentation. Those brown patches are harder to correct than whatever you came in to treat, and they can be permanent.

In practice that means staying indoors and away from windows early on, then broad-spectrum SPF 50 every day once we clear you to apply it, reapplied through the day, with a wide-brimmed hat. This holds on cloudy days and through winter. It also rules out beach holidays, ski trips and tanning for the period we specify. If you have something booked, tell us at the consultation and we will schedule around it.

It depends on what is being corrected and how deep we can safely go. Some anti-aging resurfacing goals are met in a single deeper session. Deep acne scarring generally needs a series spaced several weeks to a few months apart. The doctor gives you a realistic range at the consultation rather than a promise, because the plan gets adjusted based on how your skin responds to the first treatment.

It depends on the area treated and the depth required, since a small spot treatment and a full-face deep resurfacing session are not the same procedure. The ranges on this page reflect what the treatment typically costs across Montreal, running from around $300 for small areas to $2,500 for full-face deep anti-aging work.

Reach and recovery. Microneedling and chemical peels work at or near the surface, need a course of sessions, and cost you little or no downtime. They are the right answer for early texture change, dullness and mild unevenness.

CO2 resurfacing removes tissue and reaches the dermis, which is why it corrects damage the gentler treatments cannot touch, and why it takes a week of your life to recover from. If your concern is mild, the gentler route is the better trade. If you have already tried it and the improvement never held, that is usually the signal you need this instead.
